Hereunder are the Notices of Loss of Accountable Forms furnished to this Bureau by the LGUs pursuant to COA Circular No. 84-233, dated 21 August 1984: Loss of One (1) Duplicate and One (1) Triplicate Copy of Accountable Form No. 51 bearing Serial Number 0009998 for Brgy.
